    Regarding number 4, are you speaking of story level tests? TDD usually refers to unit tests and are written by the programmer in a tight feedback loop: write a test, watch it fail, make it pass, refactor.

    Story level test-driven development, has testers and business people writing tests as a specification of what the production code must do.

    Regarding number 5, the production code tests the test code in most cases. It’s also a good idea to specifically write tests for test-doubles to document how they work and make sure they work that way.
